Advanced care planning enables you to consider important decisions about their future health ahead of time, and letting others know- both your family and healthcare providers. Advanced Care Planning helps others know what type of medical care you want. This may include: creating an advanced directive, learning about the legalities of assigning a Power of Attorney (POA) and substitute decision makers (SDMs).
